  • As a follow-up to all emergencies, non-emergency accidents or incidents; an Accident & Incident Report needs to be completed.
  • The form can be found at Online Requests, Forms & Documents

Accident & Incident Reports completed by

  • the person injured or affected (if possible)
  • the person reporting the accident & incident
  • the person in charge at the scene
  • the affected person’s immediate supervisor

Copies of the form are distributed to

  1. Security (2E.14)
  2. Director of Facilities Management (2E.21)
  3. Director of Student Services (4E.2)
  4. Health Services (2D.2)
